Background: Minnesota Common Report Form
The Minnesota Common Report Form provides a standardized format for a nonprofit grantee to use in reporting to different grantmakers about work it has accomplished with their grants, helping to make the reporting process more efficient and effective for nonprofits and funders alike. For grantees, the form can help reduce the amount of time they must spend rearranging basic information to fit funders’ varying reporting requirements. For grantmakers, the form can help strengthen and streamline their post-grant assessment process.

The Minnesota Council on Foundations introduced the Common Report Form in March 2001. The form was developed based on input from Council members and representatives from the state’s nonprofit community. The Minnesota Common Report Form was inspired by the success of the Minnesota Common Grant Application Form, which the Council first released in 1996.

    Grantmakers Accepting the Minnesota Common Report Form

    The funders listed below have agreed to accept the Minnesota Common Report Form. Keep in mind that every grantmaker has different reporting guidelines and priorities, as well as different deadlines and timetables. Before submitting this report form to a potential funder, it is very important that you check to see when, if at all, the grantmaker would like a report, and whether there is additional information the funder requests when reporting.
